In high school, Adrian Smith played basketball for Farmington (Farmington, Kentucky). Smith played basketball in college with, Northeast Mississippi Community College (1954-1956), and Kentucky Wildcats men’s basketball (1956-1958) and started playing professionally in 1961. In the 1958 draft, Smith was chosen number 85 in round 15 by the Cincinnati Royals. Smith left the game of basketball in 1972.

Smith played for the USAF All-Stars during 1959-1960 as well as the Akron Wingfoots in 1961.
